Les associations d'usagers ont fait irruption dans le paysage français de la santé mentale depuis moins d'une dizaine d'années. Leur présence, aux côtés des associations de parents de patients, dans les conseils d'administration des hôpitaux, dans les groupes de travail du ministère de la Santé, auprès des Agences régionales d'hospitalisation, a bouleversé le fonctionnement des institutions de soins. Le législateur, par la loi du 4 mars 2002, a entériné ces évolutions. Si certains pensent que l'émergence récente et la légitimité forte des associations d'usagers sont une construction des pouvoirs publics pour favoriser l'évolution du système de santé, nous ne pouvons que constater que ces associations de patients et de familles de malades ont rapidement pris leur autonomie, dépassé le seuil que les pouvoirs publics leur auraient attribué et sont devenues de véritables moteurs de changements. Les psychiatres publics, depuis plusieurs années, ont noué un dialogue avec elles, afin de peser sur les pouvoirs publics et d'organiser un système de santé mentale plus efficient, davantage centré sur l'usager.
