Abstract

Parkinson's disease is diagnosed by history and physical examination and there are no laboratory investigations available to aid the diagnosis of Parkinson's disease. Confirmation of diagnosis of Parkinson's disease thus remains a difficulty. This book brings forth an update of most recent developments made in terms of biomarkers and various imaging techniques with potential use for diagnosing Parkinson's disease. A detailed discussion about the differential diagnosis of Parkinson's disease also follows as Parkinson's disease may be difficult to differentiate from other mimicking conditions at times. As Parkinson's disease affects many systems of human body, a multimodality treatment of this condition is necessary to improve the quality of life of patients. Abstract

Parkinson's disease is a common neurological disease and affects 2% population of more than 65 years of age and 5% more than 85 years of age. Abstract

Personalized medicine for Parkinson's disease is a growing and emerging concept in light of recent recognition that Parkinson's is a syndromic condition affecting multiple neurotransmitter systems, as well as brain and extracranial structures. The clinical expression is, thus, heterogeneous, and presentation age can range from the 30s to the 90s, with PD in older patients being associated with significant neuropathological comorbidity as well, involving not just misfolded alpha synuclein deposition but also amyloid and tau. Traditional and largely guideline-driven "one size fits all" management strategies adopted in clinical practice are, therefore, often inadequate in holistic management of a patient, particularly when aspects of motor and nonmotor symptoms are taken into consideration. In this supplement of JPM, we present a selection of papers which address several possible strands of personalized medicine in PD, ranging from genomic precision medicine to digital "checklists" to ensure delivery of holistic personalized medicine involving nonpharmacological strategies, as well. We are soliciting any papers addressing biomarkers, genetics and pharmacogenetics, treatment or complementary therapies for personalized or individualized treatment for PD.

Abstract

Parkinson's Disease (PD) is a neurodegenerative disorder that manifests with motor and non-motor symptoms. PD treatment is symptomatic and tries to alleviate the associated symptoms through an adjustment of the medication. As the disease is evolving and this evolution is patient specific, it could be very difficult to properly manage the disease.The current available technology (electronics, communication, computing, etc.), correctly combined with wearables, can be of great use for obtaining and processing useful information for both clinicians and patients allowing them to become actively involved in their condition.Parkinson's Disease Management through ICT: The REMPARK Approach presents the work done, main results and conclusions of the REMPARK project (2011 – 2015) funded by the European Union under contract FP7-ICT-2011-7-287677. REMPARK system was proposed and developed as a real Personal Health Device for the Remote and Autonomous Management of Parkinson’s Disease, composed of different levels of interaction with the patient, clinician and carers, and integrating a set of interconnected sub-systems: sensor, auditory cueing, Smartphone and server. The sensor subsystem, using embedded algorithmics, is able to detect the motor symptoms associated with PD in real time. This information, sent through the Smartphone to the REMPARK server, is used for an efficient management of the disease.
