Abstract

The current revolution in health care profoundly affects nurses in academic and clinical practice settings. It is a challenge that entails commitment to excellence in the educational preparation and professional performance of nurses. In response to these demands, the sixth edition of the Textbook of Medical-Surgical Nursing is designed to provide the sound scientific rationale and broad clinical knowledge base that nursing students and practitioners will need. The fund of information is drawn from nursing, the physical, biological, and behavioral sciences, and research. The nursing process is the central, unifying principle of the book and provides the logical framework for its organization. In applying the nursing process to practice, the nurse learns to recognize health illness problems that can be alleviated by nursing interventions. The planning and implementation of these interventions is based on a thorough understanding of biophysical and psychosocial function and dysfu nction in acute and chronic health situations. The effectiveness of the nurse's interventions is measured against expected outcomes established at the outset, reviewed during ongoing assessments, and modified as needed. Therapeutic decisions are, of course, based on individual circumstances.The perspective of this edition reflects contemporary realities and future possibilities. This approach is evident in the highlighted emphasis on gerontological considerations and in the recognition of changing demands on health care resources. Patients in hospitals, as well as those released to step-down units, extended care facilities, or their own homes, are in the main acutely ill. Because the clinical situations of persons in all of these settings are acute and can change rapidly, health-compromising complications are a major concern. Abstract
