Abstract

This volume covers the latest research and development in the areas of Vagus Nerve Stimulation (VNS) as it relates to bioelectronic medicine from neonate to adult. The chapters in this book cover topics such as invasive and non-invasive VNS including methodological considerations (study design, stimulation parameters, and use of heart rate variability metrics); mechanisms of action (automatic regulation and immune plasticity); and disorders where VNS approaches may be therapeutic (migraine and cluster headaches, mood disorders, trauma-related disorders, and language learning). In the Neuromethods series style, chapters include the kind of detail and key advice from the specialists needed to get successful results in your laboratory. Comprehensive and thorough, Vagus Nerve Stimulation is a valuable resource for both novice and expert preclinical and clinical scientists, clinicians, physicians, and scholars who are interested in learning more about this exciting and developing field.

Abstract

Edited by clinicians who were involved with transcranial magnetic stimulation (TMS) from the beginning, Transcranial Magnetic Stimulation: Clinical Applications for Psychiatric Practice offers everything the mental health practitioner needs to know about this innovative and well-established treatment. It is increasingly clear that different combinations of biological, neurobehavioral, and symptomatic factors contribute to the problem of'treatment resistance'in psychiatric disorders. Fortunately, a number of neuromodulation approaches, including TMS, are providing more options for clinicians to combat psychiatric problems. However, guidance about how to identify patients who are good candidates for TMS and how to comanage them during treatment is scarce because instruction on this modality has yet to be integrated into most psychiatry residencies. Thus, this text fills a great need, providing clinicians with an evidence-based foundation for the efficacy and safety of TMS.Despite the rapid growth of this innovative option, many practitioners are unclear about how best to utilize TMS. The book addresses these clinical concerns systematically and thoroughly: • Clinical vignettes illustrate how to identify appropriate patients for referral to a TMS clinician.• Discussions of treatment resistance, psychiatric and medical comorbidities, and preparation of the patient for TMS are included.• Because TMS is likely to be used concurrently with other treatments, the book explains how to best integrate this modality with psychotherapy, pharmacotherapy, and other forms of neuromodulation to improve outcomes.• In-depth coverage is provided on how to coordinate efforts between the primary treatment and TMS teams to assure the best outcomes during acute, continuation, and maintenance treatment.• Chapters provide a review of topic-specific literature, as well as clinical vignettes that highlight how to integrate TMS into patient care. • Key clinical points summarize the optimal clinical application of TMS for the general mental health provider.• The evolving nature of TMS research, such as the ongoing development of this and related technologies, as well as its expanding use as a potential treatment for other clinical neuropsychiatric conditions, is also addressed. Transcranial Magnetic Stimulation: Clinical Applications for Psychiatric Practice guides the general psychiatrist and mental health clinician on how to integrate this treatment modality into their practice by presenting an update on the current clinical role of TMS and a road map to its potential future.

Abstract

Written by renowned authorities in the field, Deep Brain Stimulation in Neurological and Psychiatric Disorders discusses today’s most current and cutting-edge applications of DBS. Initially used to treat Parkinson’s disease and essential tremor, DBS now has new clinical indications, new anatomic targets, and new technologies that enable an expanded role for DBS in the treatment of other movement disorders such as dystonia, and for other neurologic disorders such as epilepsy, Tourette syndrome and cluster headache. Early experience has also been reported for psychiatric syndromes, such as obsessive-compulsive disorder and depression. This important book begins with reviews of the functional anatomy and physiology of motor and nonmotor aspects of the basal ganglia and their connections which underlie the application of DBS to neurological and psychiatric disorders. This is followed by proposed mechanisms of action of DBS based on functional neuroimaging and neurophysiologic studies in animals and man. Discussions of patient selection, preoperative assessment, operative complications and brain targeting are followed by chapters concerning the demands of microelectrode mapping as well as new and emerging brain imaging approaches for target localization inside the operating room. Postoperative management and outcomes are reviewed in a series of chapters concerning immediate and delayed complications. Particularly important chapters on programming, DBS safety with regard to MRI and other electronic devices, neuropsychological sequelae, and quality of life are also included. Finally, new areas of DBS application are addressed by experts with experience in Tourette syndrome, epilepsy, obsessive-compulsive disorder, depression, and cluster headache. Comprehensive and state-of-the-art, Deep Brain Stimulation in Neurological and Psychiatric Disorders provides a balanced and level-headed approach to the use of DBS with emphasis on proper and objective assessment of outcomes, particularly in regard to the new and emerging applications -- all with the main goal of providing patients with some measure of relief from their difficult conditions.

Abstract

The preceding decade has witnessed tremendous progress in clinical as well as theoretical neuroscience. In its wake, powerful new instruments of neuromodulation acting directly on the brain have been developed: potent neuro-pharmaceuticals, deep brain stimulation, transcranial magnetic stimulation, various methods of neurotransplantation, central neural prostheses, and others. However, few areas of scientific development seem to exhibit as close a connection between dreams of progress and nightmares of disaster as contemporary neuroscience. This Janus-faced character relates to the core function of the brain as the "organ of the mind". Methods of intervening in the brain easily draw the suspicion of possibly altering fundamental features of personhood, thus posing a threat to preconditions of human dignity and to the normative structure of our societies. This suspicion has been fuelled by the discovery that most of the newly developed neurotherapeutic instruments might also be used for purposes of enhancement of certain mental features. "Mind doping" is a populist slogan at hand, suggesting a deprecatory parallel to the practice of doping in sports. The present book subjects the whole range of questions associated with these problems to a thorough exploration. Extensive state-of-the-art accounts of the relevant clinical and theoretical neurosciences are followed by an in-depth philosophical analysis of the problems of personal identity and a comprehensive disquisition on legal and ethical questions posed by present and foreseeable future practices of neuroenhancement. A concluding chapter presents the study’s main results as recommendations, addressing clinical practitioners and researchers in the field as well as to politicians, legislators, law courts, philosophers, lawyers, and anybody fascinated by or concerned about the dawning era of intervening in the brain.

Abstract

The amputation of a limb is a surgical intervention used as a last resort to remove irreparably damaged, diseased, or congenitally malformed limbs where retention of the limb is a threat to the well-being of the individual. The procedure traumatically alters the body image, but often leaves sensations that refer to the missing body part, the phantom limb. In 50-80% of cases, these sensations are perceived as painful and referred to as Phantom Limb Pain'.Direct Nerve Stimulation for Induction of Sensation and Treatment of Phantom Limb Pain provides an overview of research, experiences and results for the design, development and test of hardware and software components, and the ambition to safely implant and evaluate a novel neural interface system to combat phantom limb pain in an amputee volunteer subject.
