Abstract

AbobotulinumtoxinA (aboBoNTA) is a botulinum toxin that blocks nerve activity in the muscles, causing a temporary reduction in muscle activity. The recommended starting dosage of aboBoNTA for pediatric patients with lower-limb spasticity (LLS) is 10 U/kg to 15 U/kg for unilateral lower-limb injections, or 20 U/kg to 30 U/kg for bilateral lower-limb injections per treatment session. Treatment can be administered to multiple sites of muscles within the lower limb. If the treatment effect from the first dose diminishes over time, patients can be re-treated after a period of at least 12 weeks. The manufacturer submitted a cost-utility analysis comparing aboBoNTA with onabotulinumtoxinA (onaBoNTA) in pediatric patients two years of age or older with unilateral or bilateral LLS.

Abstract

The Knowledge Centre in the Norwegian Institute of Public Health was asked by physiotherapist Irene Krystad at the department for physiotherapy at Tysvær municipality, to find research on the effects of stretching after treatment with botulinum toxin for adult patients with spasticity. After having searched for literature, they had found limited documentation and saw the need of mapping existing research on this subject in a systematic way. We conducted a systematic mapping review by searching for systematic reviews and randomised controlled trials om the effect of stretching after treatment with botulinum toxin versus treatment with botulinum toxin alone on adults with spasticity. The search for systematic reviews resulted in 1109 references. We included four of these systematic reviews. The reviews included five randomised controlled trials with a total of 119 patients that were relevant to our research question. The search for randomised controlled trials resulted in 2617 references. Of these we included five trials with a total of 138 patients. Four of these trials were included in the systematic review. We chose not to include the fifth trial that was included in the reviews, because we considered the intervention used not to achieve a mechanism of stretching. We identified gaps in the existing literature on research on the effect of stretching after treatment with botulinum toxin versus botulinum toxin alone.

Abstract

The book is devoted to the neurosurgical management of spasticity. Starting with a chapter on the anatomical and physiological foundations of spasticity and a short history of its neurosurgical treatment, it describes the neurosurgical methods currently available. As management differs between adults and children, the book is also structured accordingly, including evaluation, decision-making, Intrathecal Baclofen Therapy (ITB), botulinum toxin therapy, and surgery. Beyond ITB, the neurosurgical options covered include procedures focusing on the peripheral nerves, dorsal roots, Dorsal Root Entry Zone and spinal cord. Based on surgical experience collected with more than a thousand patients, the book gathers the most important aspects of our present understanding, presented using a practical, educational approach. It stresses the importance of a multidisciplinary approach, including neurologists, pediatricians and rehabilitation specialists. Close collaboration with other surgical disciplines like orthopedic surgery and neuro-urology are also outlined.

Abstract
